Colon cancer (CC) is one of the most frequent neoplasms in most countries. It is a common and lethal disease associated with both environmental and genetic factors, with evasion of programmed cell death (apoptosis) and blockade of immune mechanisms being some of the essential changes in malignant transformation.
Our hypothesis is based on the fact that newly diagnosed CC patients show significant differences in the degree of lymphocyte apoptosis compared to healthy subjects and that due to the important role of the LTγδ population as the first line of intraepithelial defence in the mucosa against infection and cancer, it is to be expected that this population may be diminished in peripheral blood of CC patients. Colon cancer (CC) is one of the most frequent neoplasms in most countries. It is a common and lethal disease associated with both environmental and genetic factors, with evasion of programmed cell death (apoptosis) and blockade of immune mechanisms being some of the essential changes in malignant transformation.
Our hypothesis is based on the fact that newly diagnosed CC patients show significant differences in the degree of lymphocyte apoptosis compared to healthy subjects and that due to the important role of the LTγδ population as the first line of intraepithelial defence in the mucosa against infection and cancer, it is to be expected that this population may be diminished in peripheral blood of CC patients. This would imply an immunosuppression that would facilitate the onset or development of the disease.
Thus, our hypothesis is that patients with colon cancer might present an immunological signal differentiated from healthy subjects at the level of frequency and apoptosis in LTγδ populations in peripheral blood.
The primary objective was to measure T-αβ and T-γδ lymphocyte subpopulations and their degree of apoptosis in peripheral blood of colon cancer patients and compare them with the healthy (control) population. The secondary objectives were to relate T-αβ and T-γδ lymphocyte subpopulations and their degree of apoptosis to different clinical-histological tumor characteristics of the patients.
We had a study population of 144 subjects. For the analysis, baseline blood samples were obtained from 96 patients recently diagnosed with CC (stages I to IV), with no previous pharmacological or surgical treatment, and 48 healthy volunteers, matched for sex and age, whose tissues were healthy. After informed consent, the pathology report after surgery was obtained from all patients with CC, from which clinical and histopathological variables were collected.
Subsequent flow cytometry analysis of αβ and γδ T lymphocytes was performed, where we analyzed the percentages and apoptosis of αβ and γδ T lymphocytes.
In the statistical analysis we applied the Kolmogorov-Smirnov test for normality, if the hypothesis of normality was accepted, we used the Student’s t-test to compare means, if the hypothesis of normality of the quantitative variable was not accepted, we used the non-parametric Mann-Whitney U test with a significance level of p < 0.05.
We found a decrease in LTγδ in individuals with CC, more pronounced in patients with microvascular invasion, a history of cancer and synchronous cancer.
All αβ and γδ T lymphocyte subpopulations show an increase in the percentage of cells undergoing apoptosis compared to healthy volunteers. We observed a non-significant increase in apoptosis in stage II patients in both αβ and γδ populations.
Conclusions are that the deficit of LTγδ found in CRC patients indicates that it is related to the onset or progression of CRC and the increased percentage of cells undergoing apoptosis in all T αβ and γδ lymphocyte subpopulations, compared to healthy volunteers, suggests immunosuppression by cancer cells.El cáncer de colon (CC) es una de las neoplasias más frecuentes en la mayoría de los países. Es una enfermedad común y letal asociada tanto a factores ambientales como genéticos, siendo la evasión de la muerte celular programada (apoptosis) y el bloqueo de los mecanismos de inmunidad, algunos de los cambios esenciales de la transformación maligna.
Nuestra hipótesis se basa en que los pacientes con CC recién diagnosticados presentan diferencias significativas en el grado de apoptosis linfocitaria respecto a sujetos sanos y que debido al importante papel que tiene la población de LTγδ, como primera línea defensa intraepitelial en las mucosas contra infecciones y cáncer, es de esperar que esta población pueda estar disminuida en sangre periférica de pacientes de CC. Lo que implicaría una inmunosupresión que facilitaría el inicio o el desarrollo de la enfermedad.
Así nuestra hipótesis es que los pacientes con cáncer de colon podrían presentar una señal inmunológica diferenciada de los sujetos sanos a nivel de frecuencia y apoptosis en las poblaciones LTγδ, en sangre periférica.
El objetivo principal fue medir en sangre periférica de pacientes con cáncer de colon las subpoblaciones linfocitarias T-αβ y T-γδ, y su grado de apoptosis, comparándolas con la población sana (control). Los objetivos secundarios fueron relacionar las subpoblaciones linfocitarias T-αβ y T-γδ y su grado de apoptosis con diferentes características clínicas – histológicas tumorales de los pacientes.
Contamos con una población de estudio de 144 sujetos. Para el análisis se obtuvieron muestras de sangre basal de 96 pacientes recién diagnosticados de CC (estadios I a IV), y sin tratamiento previo ni farmacológico ni quirúrgico y 48 voluntarios sanos, apareados por sexo y edad, cuyos tejidos eran sanos. Se obtuvo de todos los pacientes con CC, previo consentimiento informado, el informe patológico tras la cirugía, de los que se recogieron las variables clínicas e histopatológicas.
Se realizó un posterior análisis mediante citometría de flujo de los linfocitos T αβ y γδ, donde analizamos los porcentajes y apoptosis de los mismos.
En el análisis estadístico aplicamos la prueba de normalidad de Kolmogorov-Smirnov, si se aceptaba la hipótesis de normalidad utilizamos la prueba t de Student para comparar las medias, en caso de no aceptarse la hipótesis de normalidad de la variable cuantitativa se utiliza la prueba no paramétrica U de Mann-Whitney con nivel de significación de p < 0,05.
Hemos encontrado una disminución de los LTγδ en las personas con CC, disminución más pronunciada en pacientes con invasión microvascular, antecedentes de cáncer y cáncer sincrónico.
Todas las subpoblaciones linfocitarias T αβ y γδ presentan un aumento del porcentaje de células que se encuentran en apoptosis, respecto a los voluntarios sanos.
Observamos un aumento no significativo, de la apoptosis en los pacientes con estadios II, tanto en las poblaciones αβ como en las γδ.
Como conclusiones obtenemos que el déficit de LTγδ que encontramos en pacientes con CCR, indica que está relacionado con el inicio o con la progresión del CC y el aumento del porcentaje de células que se encuentran en apoptosis en todas las subpoblaciones linfocitarias T αβ y γδ, respecto a los voluntarios sanos, sugiere una inmunosupresión por parte de las células cancerígenas.
